To use Acrobat Reader to fill-in and print a PDF form, please follow the instructions below:

Click on the Hand Tool icon and position the hand pointer over a line on the application so that the pointer changes into an I-beam and click once.  The blinking I-beam pointer indicates the form is ready for you to begin typing on that line.  Use the Zoom Tool, if necessary, to zoom in to the appropriate space on the form.   You may use your mouse to select another line, or press the Tab key to accept your input and move to the next form line. Press Shift + Tab if you want to move to the previous line.   Before printing your completed form, use your mouse to click on an area of the form that is outside a form field. If a form field is active (contains the blinking I-beam pointer) the contents will not print.   To print the form, click on the Acrobat toolbar printer icon -- not the browser print button -- or select File/Print... from the main toolbar at the top.
